CHOIR FORMS

There are 4 forms that must be signed and brought back to us. These forms along with the choir fees, are due NO LATER than September 4th. Please email them, upload them to Canvas, or have your child bring them in by September 4th.

They are:

-Choir Handbook Form 

-Medical Release Form

-DPISD Student Code of Conduct - this may have already been done during registration. 

-Bus Form 

 

Also, everyone will be receiving a NEW choir shirt this year. 

 

 

Cash or check made out to FJH. You can also pay via My School Bucks. Look for your child's grade level.
